MAURICE RAVEL HISTORIA MEDICA

Querida Tatiana:

      Veo que estas empezando la guardia
en el hospital universitario, mira, encontré
esto, es sobre la historia médica de Maurice
Ravel, por lo que describe Alajuanione, parece
que Ravel sufrió de una enfermedad de Pick,
se especula en el artículo, si fue una encefalopatía
espongiforme, pero no parece porque el deterioro
aunque fué tan rápido y no estuvo acompañado de
mioclonías, Ravel medía un metro con 60 centimetros,
tenía la cabeza grande con prominentes huesos
frontales que los llegaron a considerar como indicio
de hidrocefalia. Finalmente nunca se corroboró el
diagnóstico por que los familiares no permitieron
la autopsia. También tuvo un TCE con pérdida de
conciencia al ir en un taxi y a partir de entonces su
condición de deterioro mental se hizo más severa y rápida,
por lo que no podría descararse un hematoma subdural
crónico, o más bién un higroma.


Bueno, aquí tienes material bibliográfico para una "sabrosa"
sesión bibliográfica acompañada de un maravilloso vino
chileno, con quesito y dátiles.

Maravilloso video, lo abrí en un turno de 28 horas con mis residentes, fue
un refresco para el alma, muchas gracias.
A proposito del Señor Ravel , el maravilloso Maurice Ravel, se enfermó de
afasia verdad?, sería un stroke o una afasia primaria? , lo increible es
que parece que siguio componiendo, es decir no tenía amusia, ¿alguien sabe
de esto?, cuanto aporte a la humanidad a pesar de su enfermedad.

EVALUACION CLINICA DEL VERTIGO

Bueno, no creo que el tema del vértigo sea una manifestación
clínica que se sobre-simplifique a hacer resonancia o no...
 Sobre todo si es posicional y violento, en un medio donde
 sobran los recursos hacer una  RM no es problema, pero
 en nuestro país la ruta crítica podría marcar otros
caminos que ahorrarían muchos recursos a nuestros
 pacientes.

Por ejemplo, una evaluación clínica sobre inicio, curso
 clínico, relación con los cambios de posición y el examen
 clínico en especial la maniobra de Nylen-Barany y la
 presencia de nistagmus rotacional, generalmente
quita dudas respecto al origen periférico del problema ,
 en especial el vértigo paroxístico benigno.
El otro aspecto de suma importancia es el seguimiento
clínico ante y ante la menor sospecha proceder a los
estudios de neuroimagen.
Les anexo un artículo en la revisón de los cincuenta
que les envie.

En mi opinión lo más importante es determinar si hay o no causa
orgánica, tiene tumor o no? lo demás no es grave la mayoría de ls
veces, es vascular??
Yo lo primero que hago es IRM simple y contrastada de cráneo
con zoom a la base específicamente a los ángulos PC y tallo,
si descarto tumor, habitualmente responden bien a inhibidores
de recaptura de serpotonina, ansiolíticos y antivertiginosos por
8 a 10 días, luego solamente ansiolítico, relajante y IRS.
